(The Medical History Update form is for existing patients. For new patients to our practice, please fill out the New Patient Paperwork instead.)

Why do we need this information? You'd be surprised at how interconnected the human body is. You might have had a surgical procedure and need to take antibiotics before coming in to see us. Or you might be on a medication that has known drug interactions. We need to know if you have certain conditions or allergies, as well as all medications you take, so that we can provide you with the best possible care. We try not to overload you with paperwork, but we do need this information in writing.
